High School Transitions
We help high school students with disabilities transition from high school to adulthood.
What We Do
High School Transitions
Our program provides transition services to high school students that have disabilities, ages 18-21. We teach our students necessary life skills including social skills and job training to communication skills and financial management.
Supported Community Connector
Providers assist with a variety of activities to participate in while in the community. We nurture socialization, attending and participating in community events.
Supported Employment
Individuals are taught solid work habits and skills. Following directions, prioritizing and completing tasks, communication, making decisions, solving problems, how to ask for help and safety training are all parts of this service.
Prevocational
Our team helps individuals learn how to follow directions, prioritize and complete tasks, communicate, make decisions, solve problems, and how to ask for help. We also provide safety training to make sure they are properly trained for their job.
Non-Medical Transportation
Rides to and from your home for community services such as SCC, Prevocational and Supported Employment that are part of your care plan.
Residential-Family Caregiver
Individuals live in a family type setting. The provider assists with daily care; meals, medication assistance, transportation to community events and medical appointments and assists with activities of daily living.

We are Here to Provide Transitional Programs to Those in Need
Our program provides transition services, ages 18-21, like teaching adult skills, cooking, where and how to find community resources, self-advocacy, social skills, and communication skills. We also teach healthy mind and body skills, forming and maintaining friendships, and leisure time activity planning.
We also help train for skills such as money and financial management, cleaning, laundry, self-care, job resources, and interview skills. We also teach appropriate dress for an interview, resume and cover letter writing, filling out an application, follow-up, mock interviews, and job searching.
