Our Services

Supported Community Connections (SCC)

Providers assist with a variety of activities to participate in while in the community.  We nurture socialization, attending and participating in community events.

 

Volunteering, making and maintaining friendships and participating in fun activities. SCC is provided in a group setting or 1:1 as needed.  Transportation may be offered, depending on your plan.

 

Trent’s Team offers SCC to individuals who receive medicaid waivers or self pay.

Residential/Family Caregiver/ Personal Care Alternative

Individuals live in a family type setting.  This is a home environment and the provider assists with daily care; meals, medication assistance, transportation to community events, and activities of daily living. 

Prevocational Services

Individuals are taught solid work habits and skills.  Following directions, prioritizing and completing tasks, communication, making decisions, solving problems, how to ask for help and safety training are all parts of this service.  Depending on your plan, transportation may be included.  We will accompany you to your job and will make sure you understand your role and responsibilities.

Supported Employment

Typically you must first use the services of the Division of Vocational Rehabilitation before you can utilize Supported Employment Services.

Non-Medical Transportation

Rides to and from your home for community services such as SCC, Prevocational and Supported Employment that are part of your care plan.

High School Transition Services

We offer many learning opportunities to help with a smooth transition.
